Dental Network offers prepaid and indemnity/PPO dental plans for groups of 2 or
more employees/members in one of the following categories:
Employer
Paid Groups:
Employer
Paid Groups are groups with a true employer-employee relationship where employee
earnings are reported on W-2’s not 1099’s and where:
1.
the employer contributes at least 75% of the employee premiums or 50% of
the employee and dependent premiums, and
2.
at least 75% of the employees participate in one of the dental plan
options.
Voluntary
Groups:
Voluntary
Groups are any groups or associations that do not meet the requirements of an
Employer Paid Group.
