GRIEVANCE FORM

California Dental Network offers three plans tailored for individuals.  Whether you are looking for basic dental coverage or need specialty referral benefits, California Dental Network has a plan that fits your needs.  All of our individual plans offer no charge cleanings, x-rays and include Orthodontic services.

Plan 460 is designed for those who are looking for basic dental coverage with the lowest monthly cost.  Plan 460 can provide you with basic dental coverage starting at $7.95 a month.  For more information, have a look at our Plan 460 brochure.

Plan 411 is designed for those who need to save on procedures that typically cost more money, such as root canals and crowns.    Plan 411 saves you money on routine and preventive services and includes greater savings on major services.  For more information, see our Plan 411 brochure. 

Plan 595 is designed for those who are looking for the most complete coverage.  Plan 595 offers routine and preventative services with reduced cost on major services.  But it also includes cosmetic and specialty referral benefits which can save you money should you need to see a specialist.  For more information, see the Plan 595 brochure.

* Monthly rates are based on automatic checking account deductions.  Monthly premiums paid by check are subject to and additional $2.00 a month handling fee.

To enroll in a plan, follow the instructions on the "How to Enroll" page.   Completed enrollment applications received by the 20th of the month will be effective on the first of the following month.

Cancellation and Refund Policy

All requests for cancellation must be in writing and, if received by the 15th of the month, will be effective the first of the following month. All premiums are for calendar months and CDN does not prorate monthly premiums. Member who paid annual premiums will receive a refund of premiums for any calendar months remaining after CDN's receipt of their request for cancellation according to the above policy.
